Psychiatric Rehabilitation Program (PRP)

is designed to support individuals living with mental illness who face challenges in improving their quality of life and maintaining connections within their community. Offered across Maryland and regulated by the Maryland Department of Health under COMAR 10.63.01.05, PRP uses evidence-based practices and personalized support to help individuals regain stability, reduce reliance on intensive intervention services, and feel more confident and connected in their daily lives. This service is often used by individuals transitioning back into their home or community after an extended hospitalization. Adjusting to new environments or responsibilities can be overwhelming, and PRP’s in-home support helps ease this process. By offering education, skill-building, and hands-on assistance, the program reduces stress related to daily tasks and empowers individuals to take meaningful steps toward recovery.

Community Integration

These services are designed for both children and adults living with intellectual disabilities and mental health conditions. Support is provided in-home or within the community, focusing on helping individuals achieve the highest level of independence possible in their chosen living, learning, working, and social environments.

The program emphasizes the development and maintenance of essential life skills and meaningful community connections. Interventions are tailored to enhance physical, emotional, social, and behavioral well-being, while also strengthening communication and social interaction skills.

These services focus on coordinating and building both professional and natural support systems to enhance mental health care. Our team works closely with individuals to help them understand their needs and create personalized service plans.

Support may include monitoring mental health symptoms, managing medications, providing personal assistance, offering problem-solving support, developing natural support networks, and coordinating transportation to appointments.
